How do you know your program is effective: My own data and case management

What has not worked in the program any why: The students I receive for referrals often have so many other issues and needs that I do not have the resources to address all of them. Mental Health Issues are a big one with a low number of counselors in the area. Also, communities and law officials do not take the problem seriously.
